An Affidavit of Identity is a legal document that can be used to provide proof of identity to a court, bank, or business. You can make an Affidavit of Identity when you need evidence of your identity or signature. If you operate a business, you may also use this form if you would like to verify the identity of a customer. In the Affidavit, the person proving their identity (the "affiant") will typically need to provide personal information such as their legal name, date of birth, and home address. In addition to swearing that the information given is true and correct, the affiant will need to sign the document in the presence of a notary public.
