What is an Affidavit of Identity?

An Affidavit of Identity is a legal document that can be used to provide proof of identity to a court, bank, or business. You can make an Affidavit of Identity when you need evidence of your identity or signature. If you operate a business, you may also use this form if you would like to verify the identity of a customer. In the Affidavit, the person proving their identity (the "affiant") will typically need to provide personal information such as their legal name, date of birth, and home address.
